- You must have an exemption form on file with the school. Upon registering your child for school, you will need to get this form notarized. Affidavit of Religious Objection to Immunization. You can go to any notary, there are notaries offered at most UPS stores but you should call ahead to be sure the notary is in. It will cost you about $11.
- A school requirement is also that the student have filled out by a healthcare professional fill out form 3300, Certificate of Vision, Hearing, Dental, & Nutrition Screening for school admission.
- Lastly, you must also have on file with the school form 3231, Certificate of Immunizations.
